SMART would like to thank the Dental Providers and Orthodontists who have accepted our dental plan over the years with minimal out of pocket expense. They stood with us so we will continue to stand with them, now with improved benefits.
Many of the practitioners we refer to as “participating” are not in the Empire Plan.
These are Dentists, Orthodontists, Vision specialists, etc. who have agreed to accept the allowable schedule of
benefits as either payment in full or at a reduced rate for SMART members. Occasionally, providers leave the plan.
We ask that you confirm participation before making an appointment.
